Welcome to Cedar Springs Presbyterian Church’s Benefits Enrollment Webpage!! 

Use this webpage to learn about the rates and details of the benefits offered by the church.  There are also links to apps and portals for your convenience as well.  Don’t skip the Introduction video or PowerPoint and work your way through the rest of the options.    

  • Fulltime employees will be provided with a “forms packet” with additional guidance. 
  • Eligible part-time employees working at least 20 or more hours per week should skip the sections related to medical, dental, vision, HSA, and FSA benefits.  Use the enrollment and change forms provided on the webpage. 

 You may submit your enrollment, change, and/or waiver forms using either of the following methods: 

  • If, and ONLY if, you have a cspc.net email address – you may submit your forms via email to elizajones@cspc.net 
  • You may also put your forms in a sealed envelope and leave them in Eliza Jones’s mailbox. 

Eliza Jones, the Human Resource Manager is available by appointment to answer any questions or concerns that you may have after reviewing the information provided here and in any “forms packet” provided to you.  Request an appointment by emailing elizajones@cspc.net.

EPC 2021 Medical/Prescription Rates

NOTE: To determine how much you would pay for medical insurance per pay period, multiply the number the calculator spits out by 12 months, divide by 26 bi-weekly pay periods, and multiply by 20% (or 0.2). Or, for the sake of simplicity, multiply the number it spits out by 6 and divide by 65.

3 – HSA and FSA Plan Management

NOTE: If you already have an HSA or FSA account and want to continue to make contributions to it in 2021, you must fill out an Election Agreement form even if the contribution amount will stay the same for 2021.

Is my preferred dental or vision provider in-network?

To find out, visit the BCBST Website using the button below.

Click on “Find Care” just under the picture of the smiling woman.

To search for dental providers, select the “Dental” network.

To search for vision providers, select the “Vision” network.

You can login or register for an online account at the BCBST Website using the button below.
